A review of liver fibrosis and cirrhosis regression

Cirrhosis has traditionally been considered an irreversible process of end-stage liver disease. With new treatments for chronic disease, there is regression fibrosis and cirrhosis, improvement in clinical parameters (i.e. function hemodynamic markers, hepatic venous pressure gradient), survival rates, demonstrating that fibrolysis are a dynamic moving two directions. Microscopically, hepatocytes push into thinning fibrous septa with eventual perforation leaving behind delicate periportal spikes the portal tracts loss veins. Obliterated veins during progressive cirrhosis due to parenchymal extinction, vascular remodeling thrombosis often leave bile duct artery within tract. Traditional staging classification systems focused on linear, process; however, Beijing system incorporates both bidirectional nature progression fibrosis. However, even regression, lesions/remodeling, extinction cumulative mutational burden place patients at increased risk developing hepatocellular carcinoma should continue undergo active surveillance. It more appropriate consider as another stage evolution disease rather than end-stage, state.
